Swine flu guidelines for people with HIV released by CDC
The CDC on Thursday issued “interim guidance” for clinicians treating patients with swine flu who are also HIV-positive.

The document notes that people with compromised immune systems, including people with HIV, are more at risk for seasonal flu, and thus are more likely to be at risk for H1N1, or swine-origin, flu as well.
There are insufficient data available at this point to determine who is at higher risk for complications of swine-origin influenza A (H1N1) virus infection. However, adults and adolescents with HIV infection, especially persons with low CD4 cell counts, are known to be at higher risk for viral and bacterial lower respiratory tract infections and for recurrent pneumonias.
Evidence that influenza can be more severe for HIV-infected adults and adolescents comes from studies among HIV-infected persons who had seasonal influenza; these data are limited. However, several studies have reported higher hospitalization rates, prolonged illness and increased mortality, especially among persons with AIDS. Thus, immune compromised persons, including HIV-infected adults and adolescents and especially persons with low CD4 cell counts or AIDS can experience more severe complications of seasonal influenza and it is possible that HIV-infected adults and adolescents are also at higher risk for swine-origin influenza complications.
People with HIV who contract swine flu should be treated with antiviral medications zanamivir or oseltamivir, the two drugs to which the virus is not resistant. “No adverse effects” have been reported in people with HIV who have received the two flu medications and “there are no known absolute contraindications” for people receiving the drugs when they are also taking antiretroviral drugs to treat HIV, according to the CDC.

During your visit to an area affected by swine Influenza
Monitor the local situation
- Pay attention to announcements from the local government.
- Follow local public health guidelines, including any movement restrictions and prevention recommendations.
Practice healthy habits to help stop the spread of influenza
- Wash your hands often with soap and water. This removes germs from your skin and helps prevent diseases from spreading.
- Use waterless alcohol-based hand gels (containing at least 60% alcohol) when soap is not available and hands are not visibly dirty.
- Cover your mouth and nose with a tissue when you cough or sneeze and put your used tissue in a wastebasket.
- If you don’t have a tissue, cough or sneeze into your upper sleeve, not your hands.
- Wash your hands after coughing or sneezing, using soap and water or an alcohol-based hand gel.
- Follow all local health recommendations. For example, you may be asked to put on a surgical mask to protect others.
Seek medical care if you feel sick
- If you become sick with a fever plus a cough and sore throat or have trouble breathing, seek medical care right away. Tell the doctor if you have had contact with a sick person or farm animals while traveling.
- Antiviral Medications: You may want to ask your doctor for prescription antiviral medications to take on your trip as a precaution since the seasonal influenza vaccine is not expected to protect against infection with swine flu viruses. CDC recommends two prescription influenza antiviral drugs to treat and/or prevent swine flu. The drugs are oseltamivir (brand name Tamiflu®) or zanamivir (brand name Relenza®). Both are prescription drugs that fight against swine flu by keeping flu viruses from reproducing in your body. These drugs can prevent infection if taken as a preventative. If you get sick, they can make your illness milder and make you feel better faster. They may also prevent serious health problems from developing. For treatment, the drugs work best if started within 2 days of getting sick. Talk to your doctor about correct indications for treatment or prevention. Always seek medical care if you are severely ill.
- You should avoid further travel until you are free of symptoms, unless traveling locally for medical care.

3 vets now HIV-positive after VA clinic mistakes

Safer or Stupid? Some Gay Men ‘PrEP’ for Sex
More than two decades into the AIDS epidemic, the well-known conundrum of condom burnout among gay men has produced a highly controversial underground practice. “Pre-exposure prophylaxis,” or PrEP, involves taking a comprehensive anti-retroviral drug, usually tenofivir, before having sex.
Some men are actually doing this because they take safer sex seriously indeed. One doctor told me of two patients who took the drug before they had protected sex with strangers as an added preventative in case a condom breaks.
But for the vast majority of gay men, PrEP means a key to not using a condom. That’s what makes it such a hot-button issue among AIDS researchers, doctors and activists. The theory behind is that the anti-retroviral drug prevents HIV from grafting itself onto healthy cells and replicating. When HIV first enters the human body, there isn’t much of it; theoretically, if those few viruses swimming around the bloodstream don’t have anywhere to go, they’ll die out–just as do millions of germs we come across every day that don’t affect us.

Lambda Legal and ARCW File Federal Lawsuit on Behalf of HIV Positive Woman Denied Surgery by Healthcare Provider
(Milwaukee, WI, February 12, 2009) — Lambda Legal and the AIDS Resource Center of Wisconsin (ARCW) filed a federal lawsuit today in the US District Court for the Eastern District of Wisconsin on behalf of Melody Rose, a 35-year-old Wisconsin woman who was denied surgery to remove her gallbladder because she has HIV.
“Discrimination in healthcare remains a major problem for people living with HIV,” said Peter Kimball, Director of the Legal Services Program at the AIDS Resource Center of Wisconsin (ARCW). “In violation of both federal and Wisconsin law, Dr. Steven Cahee refused to provide needed medical treatment to our client, Melody Rose.”
Lambda Legal and ARCW represent Melody Rose who experienced health problems with her gallbladder while incarcerated at Taycheedah Correctional Institution in Fond du Lac. Ultimately, her physician referred her to Dr. Steven Cahee at the Fond du Lac Regional Hospital to have her gall bladder removed.
However, Dr. Cahee refused to perform the surgery after learning Ms. Rose is HIV-positive. Some time thereafter, a surgeon at a different medical facility removed Ms. Rose’s gallbladder in a laprascopic procedure, which is considered routine surgery.
“Dr. Cahee’s actions fly in the face of ethical behavior for a medical professional,” said Scott Schoettes, Staff Attorney with Lambda Legal’s HIV Project. “Long ago, the Centers for Disease Control and Prevention established that using universal precautions — which are required in all sorts of medical situations, including surgeries — makes it extremely unlikely for the virus to be transmitted in this setting.”
The complaint filed today alleges that the defendants –- Steven M. Cahee, M.D., Agnesian HealthCare, Inc., and Fond du Lac Regional Clinic, S.C., violated four different state and federal laws. The complaint alleges that the defendants violated the Americans with Disabilities Act (ADA) and its state law counterpart, which both prohibit disability discrimination by places of public accommodation (including healthcare providers); the Rehabilitation Act, which requires recipients of federal financial assistance not to discriminate against people with disabilities; and a Wisconsin law addressing HIV discrimination specifically, which forbids healthcare providers from denying services to people (or humiliating and degrading them) based solely on their HIV status.
The case is Rose v. Cahee, et al.

Novel SF program tries to cut new HIV cases
This month, the San Francisco AIDS Foundation is launching an innovative program designed to catch new HIV infections shortly after they occur – when the risk of transmission is the highest.
The goal of the two-year pilot project, the first of its kind in the nation, is to reduce by half the number of new HIV cases by 2015.
During the two- or three-month period after infection, the viral load is highest and the danger of transmission is also at its peak. As many as half of all new infections are estimated to occur during the acute phase.
“The virus gets in your body and starts to replicate at a very high rate before the natural immune responses of your body start to mobilize,” said Mark Cloutier, chief executive of the San Francisco AIDS Foundation.
The expanded testing, along with counseling, will take place at Magnet, a community health center for gay men in the Castro.
Clients who report engaging in recent, high-risk behavior will be invited to take viral RNA (ribonucleic acid) testing, which will identify those who are acutely infected. They will also be encouraged to alert their partners.
It takes two weeks to get results. During that time, said Steve Gibson, director of Magnet, clients are counseled to behave prudently, as if they were HIV-positive.

Businessman offers $100M toward AIDS vaccine research
The hunt for an AIDS vaccine, a scientific quest that has stumped infectious disease researchers for two decades, is receiving a $100 million boost from a Massachusetts technology magnate, whose gift will create a Boston institute fusing the expertise of doctors, engineers, and biologists.
Stunned by scenes of desperation he witnessed in HIV-ravaged South Africa, Phillip Terrence Ragon is spending a considerable chunk of his fortune to accelerate research for a vaccine that would slow the relentless spread of the virus that causes AIDS and now infects more than 33 million people worldwide.
The money, $10 million a year for the next decade, will go to Massachusetts General Hospital but be shared with other research powerhouses, including Harvard University and the Massachusetts Institute of Technology. An announcement of the gift, the largest in Mass. General’s history, is expected this morning from Ragon, 59, the founder and sole owner of InterSystems, a Cambridge company that provides database software to hospitals and other industries.

Political Notebook: BART relocates ads featuring gay supervisor
Ads promoting a study for an HIV prevention pill that feature District 8 Supervisor Bevan Dufty have been tagged with graffiti at such a rate that BART officials recently relocated the posters at four Muni stations to better protect them.
The paid advertisements plug the Department of Public Health’s research into whether Truvada, an HIV medication, can keep HIV-negative men from contracting the virus. Dufty, who is openly gay and single, appears in one version of the ads dressed in a shirt and tie, sans jacket, but is not identified by name.
That hasn’t stopped his critics, or mere pranksters, from defacing the posters by writing in black marker “I’m easy” or “I killed Halloween,” referring to Dufty’s helping to scuttle the annual street party in the Castro on Halloween night in order to end the violence that has marred the event in past years.
